Using a disassembler

Is it possible to use the output of a disassembler to reconstruct and
patch routines into a program?  Is there something that would interpret
the assembly output into 'C' or something of the sort?

Why I ask is that there are some programs that I have in BINARY only 
form which I would like to patch routines into... without the src,
it's really difficult, if not impossible to do... so I thought some
C guru out there might offer some helpful hints, etc.
