Perhaps you can set bit 7 in your Ctrl/D and sneak it through.
I think a better idea is to support XON/XOFF in raw mode, since many
terminals absolutely MUST synchronize the transmission to them to avoid
buffer overflow (for example Anadex, VT100, Diablo, & Versabraille).  We
have put XON/XOFF support into raw mode -- it is extremely easy to do.
