Termcap entry for Concept AVT terminals

The Concept-AVT instruction set is a superset of the basic VT-100 instruction
set; the Concept-AVT's instruction set looks nothing like the Concept-100, 104,
or 108 instruction sets, nor like the Concept-LNZ instruction set.
We briefly used Concept-AVT's as VT-100's with Tops-20 Emacs before we
replaced the software in the AVT's with Concept-LNZ software.
