Locating a copy of "The Lions Document"

while we're on the subject, my copy of the source code is missing
sheets 15 and 16; is there someone who could supply me with a copy
of that page?  it's like having a first edition mikey mouse watch
minus one hand.

everybody seems to know about the lyons "book"; i have also seen
a bell labs doc titled "unix program description", published by the
unix support group, around 1976.  it contained a function-by-function
walk thru the kernel, in a modified man-page format.  the thing i
remember most about it now is that it was full of typos.
