I just received 100 copies (promised falsely by Addison-Wesley the
week after Usenix) and am now shipping the orders we took in San
Diego.  Many thanks to all for the patience shown.  We are due to
receive a second 100 by next Wednesday; I double-checked that they
are already on the way.
