In article <4764 at vice.ICO.TEK.COM> michaele at vice.ICO.TEK.COM
(Michael Enkelis) writes [edited to bare essentials]:
>uda0: soft error, SDI error, unit 0, event 053, hdr 0x0
>uda0: soft error, SDI error, unit 0, event 0113, hdr 0x0
>uda0: hard error, SDI error, unit 0, event 0113, hdr 0x0

These are, in order:

sdi command timeout drive error (soft)
ctlr detected protocol drive error (soft)
ctlr detected protocol drive error (hard)

This indicates a problem in the controller (uda50 or equivalent), drive,
or cabling between the two.

Convert to 4.3BSD-tahoe; you will get a much better UDA/RA driver.
