Xenix system crash

Greetings,
I have gotten two system crashes in the past two weeks, both similar in
nature. They both occurred after the system had been up for almost a
week, and the process id's were well of 15000 (in one case of 25000).
Each time the system was doing something different. The results of the
crash presented me with a "kernal: PANIC: double exception" message; I
have reproduced the full register dump below with the leading zeroes
removed.
