need more help with SunView

We've got a pilot system running on 3/50s (SunOS 4.0), using SunView
windows.  I've been trying for some time now to track down an error
message, and would appreciate any help I could get.

The message shows up about four times running when a bunch of processes
are being started up, and it's exactly the same each time.  It's
apparently due to a bogus ioctl.  Looking in the SunView programmers
manual led me to deciding that it was due to a WININSERT ioctl, but I sure
can't find any place in any source code where such a call is made.

If anyone can help with this problem I'll be your friend forever...
