VFS construction

Has anyone out there actually built their own VFS? All the
documentation is in place, but without an example it threatens to be a
major exercise in experimentation to see how it fits together.  For
example, does one actually have to have "directories" in standard Unix
format, or can one implement the lookup operations however one likes?
Which of the vnode/gnode ops have to work, and which can just return
errors?
