Problems with Drivers for Emulex cs21

I have saved several different drivers for the EMULEX cs21 from the
net.  Now I need to use one, and I can't get them to work.  

The driver posted by parsec!sheppard on December 15, 1983 tries to
include "../h/uio.h".  My 4.1 system doesn't have this file;
Is there something else I should use, or am I out of luck?

The drivers that sbcs!perry posted in september and that emory!km
announced last summer give the following errors when compiling:

Is this another case of needing an include file I don't have?  Can
anyone suggest something?  I'll be sending notes to the posters of
these drivers, but if you have any experience with them, please let me
know.
